- The CDC voice and fax toll free information service (888) 232-3228 offers documents about individual diseases.
- Bulletins about disease outbreaks in specific countries can be ordered from (877) FYI-TRIP.
- Health Information for International Travel (publication # CDC94-8280) contains vaccination recommendations and health-related travel tips. Contact the U.S. Government Printing Office at (2020) 512-1800.
To find out which vaccines are recommended for the country you are visiting, check www.cdc.gov/travel or consult a travel clinic.

Greendale Health Department does not provide travel immunizations. Travel immunizations are offered at the following sites. Appointments may be required and fees may apply. Call ahead to verify information as information may change without notice. In addition, your health care provider, clinic or HMO may be able to provide you with travel related vaccines.
